Please use this form to apply for lifetime registration of a cat or dog under the Companion Animals Act 1998. Please note that if you give false or misleading information you may be fined.
STEP 1 - MICROCHIPPING
Cats and dogs in NSW must be permanently identified by microchipping by the time the animal is 12 weeks old or prior to being sold or given away under the Companion Animals Act 1998. Once the animal is microchipped, these identification details are entered into the NSW Companion Animal Register. A certificate is then issued to the owner with the identification details. If this has not occurred you can a Vet or Authorised Identifier can complete a Verification of Existing Microchip Form.
STEP 2 - REGISTRATION
If you have trouble registering your pet through the petregistry.nsw.gov.au Council can assist you with your registration.
You’ll need to:
- Complete this Form
- If change of ownership has not occurred via the NSW Pet Registry you will need to provide a signed and completed a Change of Ownership form OR obtain a Statutory Declaration.
- Proof to support registration selection under the following registration types
- Desexed - a letter, certificate or clear receipt from a vet OR a statutory declaration from the owner
- Recognised breeder - proof of current membership of Dogs NSW, NSW Cat Fanciers Association, Waratah State Cat Alliance or any other body approved by the Chief Executive, and, documentation verifying the cat or dog is of a breed accepted by the recognised breeder body, and, a signed statement from the member that the animal is to be kept for breeding purposes.
- Pensioner – current pensioner concession card or other proof of status as an eligible pensioner.
- Assistance animal – a statement from a recognised assistance animal training organisation (eg NSW Guide Dog Association) that the animal is trained, or is being trained, as an assistance animal and is used as an assistance animal.
- Working Dog – the owner must satisfy council that the dog is primarily used for the droving, tending, working or protecting of stock (includes a dog being trained as a working dog)
- Registered racing Greyhound – Greyhound is registered in accordance with the rules under the Greyhound Racing Act 2009.

NOT DESEXED AND OVER 4 MONTHS OLD
- Standard Registration Fee or $65 and an annual permit is required each year the cat is not desexed in addition to the registration fee of $92.
- Recognised breeders and cats not recommended for desexing for medical reasons (by a vet) are exempt
OTHER FEES
- $21 late fee – registration not paid 28 days after the animal is 6 months of age or after the annual permit requirement took effect
NOTE:
Registration and late fees are set and charged in accordance with the NSW Pet Registry.
The Administration Fee is set as per the current Council's Fees and Charges.
A Credit Card Surcharge of 0.55% applies for processing online applications.

Port Stephens Council is committed to protecting your privacy. We take reasonable steps to comply with relevant legislation and Council policy.
Purpose: Council is collecting this information to register your animal with the NSW Pet Registry.
Intended recipients: Council staff and other government agencies.
Supply: This information is a statutory requirement.
Consequence of Non Provision: Your application may not be accepted or processed due to a lack of information.
Storage and security: This document will be saved in Council’s records management system in accordance with Council policy and relevant legislation. Your information will be entered/updated on the NSW Companion Animal Registry. Only authorised people, such as the Council staff and Police, can access the information on the NSW Companion Animal Register and only to enforce the Companion Animal Act 1998.
If you are concerned that the disclosure of your information could jeopardise the safety of you or your family, you may request that these details be suppressed. If this is the case, we recommend that you do not list your animal on any private databases. Please contact Council on 02 4988 0255.
